2026 Nebraska Medicaid Income Limits by Category

Income eligibility for most Nebraska Medicaid programs is calculated using Modified Adjusted Gross Income (MAGI) and is expressed as a percentage of the Federal Poverty Level (FPL). The 2026 FPL guidelines took effect January 14, 2026.

Heritage Health Adults (Medicaid Expansion)

Adults aged 19 to 64 who are not pregnant and do not qualify through another category can enroll through Heritage Health Adult if their income falls at or below 138% FPL.

Children (Ages 0 to 18)

Children qualify for Medicaid at a higher income threshold than adults. Nebraska covers children up to 218% FPL. This includes both traditional Medicaid and the Children's Health Insurance Program (CHIP), administered together under Heritage Health.

Pregnant Women

Pregnant women qualify at 199% FPL. Coverage begins from the date of application and extends through 60 days postpartum. Nebraska's Extended Medicaid for Pregnant Women continues full Medicaid benefits for 12 months after delivery.

Parents and Caretaker Relatives

Parents and caretaker relatives living with dependent children may qualify at 138% FPL, the same threshold as expansion adults. This category also carries no asset test.

Aged, Blind, and Disabled (ABD) Medicaid

People who are 65 or older, blind, or have a qualifying disability may qualify for ABD Medicaid. Income limits for this group are lower because the program uses different standards tied to the Social Security Substantial Gainful Activity (SGA) limits rather than MAGI. Income is generally limited to around 100% FPL for home and community-based waiver programs.

A resource (asset) limit applies to this group:

  • Single applicant: $4,000
  • Two-person household: $6,000
  • Each additional member: $25

Long-Term Care and Nursing Home

For nursing home Medicaid, Nebraska uses an income standard of approximately $2,829 per month for 2026 (300% of the SSI federal benefit rate). If income exceeds this amount, a Miller Trust (Qualified Income Trust) can be set up to redirect excess income and maintain eligibility. Asset limits are $4,000 for an individual, with the community spouse able to retain up to the Community Spouse Resource Allowance.

New Work Requirements in 2026

Nebraska became the first state in the nation to implement Medicaid work requirements under federal approval. Starting May 1, 2026, able-bodied adults applying for or enrolled in Heritage Health Adult must meet an 80-hour-per-month work activity requirement.

Qualifying activities include:

  • Employment (paid work)
  • Job training or job search activities
  • Vocational education or apprenticeship programs (at least half time)
  • Community service
  • Combinations of the above totaling 80 hours in any calendar month

Who Is Exempt from Work Requirements?

Many people are automatically exempt and do not need to complete work activities:

  • Adults 65 and older
  • Pregnant women
  • Children under 19
  • People with a qualifying disability or who are medically frail
  • Caregivers of a child age 13 or younger
  • Caregivers of a disabled family member
  • Former foster care youth who aged out and had Medicaid at that time, if under age 26
  • People enrolled in a substance use disorder (SUD) treatment program

How Income Is Counted
