Who Qualifies for Medicare in Kentucky

Medicare eligibility in Kentucky follows federal rules. Most people qualify at age 65 if they are a U.S. citizen or permanent resident who has lived in the country for at least five consecutive years. You also need to have earned 40 quarters of Medicare-covered work history (or be married to someone who has) to receive Part A premium-free.

People under 65 can qualify for Medicare if they:

  • Have received Social Security Disability Insurance (SSDI) for 24 consecutive months
  • Have been diagnosed with End-Stage Renal Disease (ESRD) requiring dialysis or a kidney transplant
  • Have been diagnosed with ALS (Lou Gehrig's disease), in which case Medicare begins immediately

If you have fewer than 40 work quarters, you can still enroll in Part A by paying a premium. In 2026, that premium is $311 per month for those with 30 to 39 quarters, and $565 per month for those with fewer than 30 quarters.

Before looking at savings programs, here is what Medicare costs without any assistance in 2026:

Kentucky Medicare Savings Programs: 2026 Income Limits

Kentucky operates three Medicare Savings Programs that help pay Medicare costs for lower-income residents. Each program has different income limits and covers different expenses.

QMB: Qualified Medicare Beneficiary

QMB is the most comprehensive program. It pays your Part A premium (if applicable), Part B premium, and all cost-sharing including deductibles, coinsurance, and copayments.

QMB covers roughly the first 100 percent of the Federal Poverty Level (FPL). Asset limits apply: $9,660 for individuals and $14,470 for couples (not counting your home, one car, or personal belongings).

Providers are prohibited from billing QMB members any cost-sharing amounts, even if Medicaid does not cover the service. If a provider bills you for a copay or deductible while you are enrolled in QMB, you have the right to dispute it.

SLMB: Specified Low-Income Medicare Beneficiary

SLMB pays the Part B premium ($202.90/month in 2026), saving enrollees about $2,435 per year. It does not cover deductibles or copayments.

SLMB covers income roughly between 100 and 120 percent FPL. Asset limits are the same as QMB: $9,660 for individuals and $14,470 for couples.

QI: Qualifying Individual

QI also pays the Part B premium. The income range is slightly higher than SLMB. Unlike QMB and SLMB, QI funding is capped each year and enrollment is first-come, first-served. You must reapply each year to maintain QI benefits, and enrollment does not carry over automatically.

QI covers income roughly between 120 and 135 percent FPL. Asset limits for QI are $9,950 for individuals and $14,910 for couples.

Asset Limits

All three programs require assets below a certain level. The following items do not count toward the asset limit:

  • Your primary home
  • One vehicle
  • Personal belongings and household items
  • A burial fund up to $1,500
  • Life insurance with face value under $1,500

Countable assets include bank accounts, stocks, bonds, and additional vehicles or real estate.

Extra Help for Part D Prescription Costs

Anyone enrolled in QMB, SLMB, or QI automatically qualifies for Extra Help (also called the Low-Income Subsidy, or LIS). Extra Help reduces or eliminates the cost of Medicare Part D prescription drug coverage, including premiums, deductibles, and copayments.

For 2026, you can also apply for Extra Help directly if your income is up to approximately $2,015 per month for individuals or $2,725 per month for couples (roughly 150 percent FPL). Annual income limits are approximately $23,940 for individuals and $32,460 for couples.

Resource (asset) limits for Extra Help are $18,090 for individuals and $36,100 for married couples (including burial expense exclusions).

IRMAA: Higher-Income Medicare Surcharges

If your income is above certain thresholds, you pay more than the standard Part B premium. This additional charge is called IRMAA (Income-Related Monthly Adjustment Amount). Your 2026 IRMAA is based on your 2024 Modified Adjusted Gross Income (MAGI).

How to Apply for Kentucky Medicare Savings Programs

Kentucky processes MSP applications through the Department for Community Based Services (DCBS). You have four ways to apply:

Step 1: Gather your documents. You will need:

  • Proof of identity (driver's license or state ID)
  • Medicare card or Medicare number
  • Proof of income (Social Security award letter, recent pay stubs, pension statements)
  • Proof of assets (bank statements, investment account statements)
  • Proof of Kentucky residence (utility bill, lease agreement)

Step 2: Choose your application method.

  • Online via KYNECT: Go to kynect.ky.gov and log in or create an account. Select "Benefits" and complete the application for health coverage assistance.
  • In person: Visit your local DCBS office. Find your nearest office using the DCBS office locator at chfs.ky.gov.
  • By phone: Call DCBS Call Services at 1-855-306-8959 Monday through Friday during business hours.
  • By mail: Download and complete the MAP-15 form from chfs.ky.gov/agencies/dms and mail it to your local DCBS office.

Step 3: Submit the application and wait for determination. DCBS typically processes MSP applications within 45 days. If approved, coverage is usually retroactive to the first day of the month you applied.

Step 4: Renew annually. QMB and SLMB benefits renew automatically if you stay enrolled in Medicaid. QI benefits require a new application each year and are not guaranteed due to first-come, first-served funding.
