Learn about North Dakota Medicaid eligibility for 2026, including income limits by household size, coverage groups, application steps, and how to check if you qualify.

North Dakota is one of the states that expanded Medicaid under the Affordable Care Act, which means more residents have access to affordable health coverage than in non-expansion states. This guide covers everything you need to know about North Dakota Medicaid eligibility in 2026, including income limits, how to apply, and what to expect.

How North Dakota Medicaid Works

North Dakota Medicaid provides health care coverage to residents who meet certain income and eligibility requirements. The program is administered by the North Dakota Department of Health and Human Services (HHS). Because North Dakota accepted Medicaid expansion in 2014, adults without children can qualify for coverage if their income falls below the limit, which is not the case in every state.

Medicaid expansion in North Dakota is managed by Blue Cross Blue Shield of North Dakota. As of October 2025, approximately 105,481 North Dakotans were enrolled in Medicaid or CHIP, including about 25,187 through ACA Medicaid expansion.

What North Dakota Medicaid Covers

North Dakota Medicaid provides comprehensive health care coverage including:

  • Doctor visits and hospital stays
  • Prescription medications
  • Preventive care and wellness visits
  • Dental care
  • Vision care and eye exams
  • Behavioral and mental health services
  • Lab tests and X-rays
  • Maternity and newborn care
  • Emergency services

Children on Medicaid receive additional benefits through the Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) program, which includes regular wellness checkups, immunizations, and developmental screenings.

Special Situations

Pregnant Women

Women who are pregnant and enrolled in Medicaid receive coverage for prenatal care, labor, and delivery. After pregnancy, coverage continues for a full 12 months postpartum, including checkups, behavioral health services, preventive care, mammograms, and dental care.

Newborns

A newborn automatically qualifies for North Dakota Medicaid if the birth mother was covered by Medicaid on the day the child was born. Coverage lasts for the first year of life.

Former Foster Youth

Young adults who aged out of foster care qualify for Medicaid until they turn 26, regardless of income.

Over-Income Options

If your income is above the Medicaid limit but you have significant medical needs, North Dakota offers a coverage option with a "client share." This means you may still receive Medicaid benefits but will need to pay a portion of your medical costs.
